This office space is located at 400 Union Ave SE, Suite 200, Office 202. When you arrive, enter through Adams street and park in the lot. Due to the fact that this is a secured building, if your appointment falls outside of regular business hours (9am-5pm Monday-Friday) on weekdays or on Saturdays, the building automatically locks and I will meet you at the outside door at the time of your appointment.

Please look at all available appointment types before selecting. If you would like a Superbill Reimbursement, please select the single Olympia Patient appointment type and not the package. The package does not qualify for reimbursement but is 25% off the total of sessions.
